Card on File Policy

Due to the increasing prevalence of high-deductible health plans and the growing portion of healthcare costs that fall to patients, St. Gianna Clinic is committed to maintaining a financially responsible and sustainable practice while continuing to provide high-quality care.

To support this, all patients are required to keep a valid credit, debit card or HSA card (Health Savings Account) on file with not to exceed a limit of $150.00 per visit.

By providing a card on file, patients authorize St. Gianna Clinic to charge the card for any patient responsibility balances, including co-payments, co-insurance, deductibles, missed appointment fees, or services not covered by insurance.

Patients will be required to sign a consent authorizing St. Gianna Clinic to charge the card on file.

To ensure transparency and respect for our patients, charges will not exceed the maximum approved by the patient without prior notification. Any balance above the approved amount will be communicated in advance to allow for approval or alternative payment arrangements.

Patients may revoke the card on file by contacting St. Gianna Clinic.

If at any time there is a data breach, St. Gianna Clinic will notify patients with a card on file within 45 days. Patients will be notified of the nature of the breach, information affected and steps you can take to protect yourself.

Patients will receive a receipt for all charges processed. All payment information is securely stored in compliance with HIPAA and PCI standards.

This policy allows us to remain good stewards of our resources, reduce administrative costs, and continue providing timely, mission driven care to all patients.
